Please complete this form if you received consultations through an Employee Assistance Program (EAP), or if your employer funded your consultations.

Your organisation is interested in evaluating our service, and have requested we provide them with feedback periodically (no more frequently than once every 6 months). We therefore supply a collated report of the comments we receive from all staff who have used COGNITION from your organisation. No names are provided in the collation, and we take care to ensure that individuals cannot be identified. We also request that you look over what you have written, before you submit feedback, to ensure you are satisfied you cannot be identified.
